Educational Grants
American Medical Systems, Inc. (AMS) supports bona fide medical education as a means of enhancing scientific knowledge, professional skills, medical advancement, and delivery of effective health care for the benefit of patients. Educational Grants include contributions to third-party academic or educational conferences or congresses, residency programs at academic institutions or hospitals such as Grand Rounds, ACCME-accredited Continuing Medical Education (CME) programs, and public or patient education programs.
Educational grants may be used by a conference sponsor to reduce legitimate expenses tied to bona fide educational activities or to allow attendance by medical students, residents, fellows, and others health care professionals in training. The educational event must be focused on promoting objective scientific and educational activities and discourse related to disease stated and treatment options for male and female pelvic health and be consistent with applicable standards established by the conference sponsor or other accrediting body.
A conference sponsor will retain independent control and is solely responsible for the selection of program content, faculty, educational methods, and materials.
Educational Grants are awarded at the sole discretion of AMS and based on AMS’ business needs. Submission of a request does not guarantee that an Educational Grant will be awarded. No Educational Grants will be awarded as an inducement for the purchase, lease, recommendation, order, or prescription of any AMS products or as a reward for previous, current, or prospective business.
Eligibility for Educational Grant
Please read the following information carefully. You must meet our eligibility requirements to be considered for a grant.
To submit an Educational Grant request, please verify that you meet the Eligibility requirements listed below.
Eligibility Requirements
- Educational Grant requests must meet one of the following areas:
- Continuing Medical Education (CME) accredited by the Accreditation Council for Continuing Medical Education (ACCME) and/or;
- Defraying legitimate expenses of educational programs that are reflective of American Medical Systems, Inc.'s (AMS) business needs or interests with particular focus on pelvic health;
- Organizations seeking an Educational Grant must be a 501(c)(3) organization or an accredited academic medical center.
Please be advised that AMS does not award Educational Grants directly to a physician licensed in the United States of America or for an educational event that has already occurred.
Request Form for Educational Grant
If you meet the Eligibility Requirements please:
- Download and fully complete the Educational Grant Request Form;
- Provide a copy of the Internal Revenue Service (IRS) Letter indicating the organization’s status as a 501(c)(3) charitable organization;
- Provide a detailed description of the intended Continuing Medical Education (CME) event or other comparable educational program including a copy of the agenda or brochure;
- Provide a list of the presenters, their qualifications, academic affiliations, and a copy of their curriculum vitae;
- Provide a list of anticipated number and description of attendees;
- Provide a copy of the Accreditation Council on Continuing Medical Educational (ACCME) certification or the equivalent for the CME or other comparable educational program;
- Submit your completed Educational Grant Request Form and supporting documentation via email to moc.dmma@tseuqeRtnarG.
American Medical Systems, Inc. (AMS) reserves the right to request additional information regarding your organization and programs or events related to your Educational Grant request.

Please visit and review the Eligibility Requirements under the Eligibility Requirements tab and then download and fully complete the Educational Grant Request Form under the Request Form tab. Submit via email the completed Educational Grant Request Form and supporting documentation to moc.dmma@tseuqeRtnarG with Educational Grant Request Form in the subject line. Only requests submitted on the Educational Grant Request Form will be considered.
How many Educational Grant Request Forms can I submit?
An organization or entity may only submit one Educational Grant Request Form for a particular educational program or event.

How quickly will a decision be made on my Educational Grant Request Form?
Please allow thirty (30) days for review of a fully completed Educational Grant Request Form. For extenuating circumstances, when a request is time sensitive, AMS will do its best to accommodate a shorter turn around time. This situation should be the exception and not the rule. A request for expedited review should be indicated on the Educational Grant Request Form.
Can I amend or change the Educational Grant Request Form that I already submitted?
No but AMS may seek clarifying information or request additional support documents.
What do I need to include with my Educational Grant Request Form?

Will I receive confirmation that my Educational Grant application was received?
Yes. You will receive an email confirmation within 1-2 days of receipt.
