Fellowship Grants

American Medical Systems, Inc. (AMS) recognizes the importance of clinical education and training as a means of advancing medical technology, treatment and knowledge, and improving patient care. In furtherance of AMS’ desire to support and foster such education and training, AMS has established a program to provide Fellowship Grants to genuine medical education of medical students, residents, and fellows participating in fellowships in urology, urogynecology, gynecology, proctology, prostate health or in a subspecialty thereof at selected tax-exempt, nonprofit teaching hospitals with accredited medical residency and fellowship programs.

Fellowship Grants are awarded at the sole discretion of AMS and based on AMS’ business needs. Submission of a request does not guarantee that a Fellowship Grant will be awarded. No Fellowship Grants will be awarded as an inducement for the purchase, lease, recommendation, order, or prescription of any AMS products or as a reward for previous, current, or prospective business.

Eligibility for Fellowship Grant

Please read the following information carefully. You must meet our eligibility requirements to be considered for a grant.

To submit a Fellowship Grant request, please verify that you meet the Eligibility requirements listed below.

Eligibility Requirements

  1. Fellowship Grant requests must:
    1. Support a Clinical Fellowship at a Qualified Teaching Hospital;
    2. The Qualified Teaching Hospital must be a 501(c)(3) or a state instrumentality under Section 115 of the Internal Revenue Code; and
    3. The Qualified Teaching Hospital must operate an accredited and/or recognized, well established medical residency and fellowship programs in urology, urogynecology, gynecology, proctology, prostate health, or some other related subspecialty as a division or affiliate of an accredited school of medicine.

Request Form for Fellowship Grant

If you meet the Eligibility Requirements please:

  • Download and fully complete the Fellowship Grant Request Form;
  • Provide materials describing the Qualified Teaching Hospital’s residency and clinical fellowship programs;
  • Provide the name(s) and curriculum vitae of the physician(s) supervising the clinical fellow;
  • Provide accreditation status of the Qualified Teaching Hospital’s medical residency and fellowship programs;
  • Provide the number of years the residency and fellowship programs in urology, urogynecology, or gynecology have been in effect;
  • Provide a description of the process by which the clinical fellow is selected;
  • Provide a copy of the Internal Revenue Service (IRS) Letter indicating the Qualified Teaching Hospital’s status as a 501(c)(3) charitable organization
  • Submit your completed Fellowship Grant Request Form and supporting documentation via email to moc.dmma@tseuqeRtnarG.

How do I apply for a Fellowship Grant?
Please visit and review the Eligibility Requirements under the Eligibility Requirements tab and then download and fully complete the Fellowship Grant Request Form under the Request Form tab. Submit via email the completed Fellowship Grant Request Form and supporting documentation to moc.dmma@tseuqeRtnarG with Fellowship Grant Request Form in the subject line. Only requests submitted on the Fellowship Grant Request Form will be considered.

How many Fellowship Grant Request Forms can I submit?
A Qualified Teaching Hospital may submit more than one Fellowship Grant Request Form, however, because of limited funds; it’s likely that AMS will only award a Qualified Teaching Hospital one Fellowship Grant at a time.

Does AMS accept paper Fellowship Grant Request Forms?
Yes, although we prefer and recommend that Fellowship Grant Request Forms be submitted electronically via email to moc.dmma@tseuqeRtnarG. Hard copies of the Fellowship Grant Request Form may be mailed to:

How quickly will a decision be made on my Fellowship Grant Request Form?
Please allow a minimum of thirty (30) days for review of a fully completed Fellowship Grant Request Form. Once a determination is made to award a Fellowship Grant, the parties must enter into a written agreement. Negotiating the terms and conditions of the agreement will take additional time beyond the thirty (30) days.

Can I amend or change the Fellowship Grant Request Form that I already submitted?
No but AMS may seek clarifying information or request additional support documents.

What do I need to include with my Fellowship Grant Request Form?
There is a description of the requisite support documents and information that should be included with the Fellowship Grant Request Form under the “Request Form” tab.

Will I receive confirmation that my Fellowship Grant application was received?
Yes. You will receive an email confirmation within 1-2 days of receipt.
